When Can You Change Your Name
Once you’re 16 years old, it’s legally your own decision to change your name. You’re treated in the same way as an adult. That means you can change your own name without your parents’ consent. Also, a 14 year old cannot change their last name officially or legally because they are not old enough to do so.
Many Courts require a child who is 14 years or older, but less than 18, to give permission for the name change. The child must sign the Minor Consent form in front of a Notary and you must give it to the Clerk with your Petition.
You can change your child’s first name once before their first birthday and once between the ages of 1 and 18. You can change your child’s family name once every 12 months.

To change your name, obtain documents that comply with the laws of your state. Complete the paperwork, but wait to sign until you are before a notary public. Publish the Notice of Petition for Change of Name.
