The REV-1605 is used to add or change corporate officer information to an existing corporate tax account in Pennsylvania. Corporations file this form to update the names of their corporate officers with the Pennsylvania Department of Revenue. The department then forwards this information to the Pennsylvania Department of State for public records. Corporations may file an updated REV-1605 during the year if corporate officer information changes.

The new 10-digit Revenue ID numbers that were recently issued to Pennsylvania businesses can be viewed within e-Tides. The Department of Revenue encourages businesses to use their new ID number in all correspondence with the Department.
